If you need an IRS Form W-9, please e-mail Executive Director Lou Miller at lmiller@njasc.org and he will e-mail or fax this form to you.
Please note that for security reasons we do NOT link to this form for direct download from our website.
NJ Business Registration Certificate
In many cases you will not need to submit this certificate to your Business Office, as under the provisions of P.L. 2004 c. 57 this requirement does not apply to "contracts paid from school student activity funds, so long as there is no contribution of public funds".

For your convenience, we are keeping active the link to our Business Registration Certificate, as we anticipate it will take some time before School Business Administrators become fully aware of these facts and thus recognize that in many cases this certificate is not really required.
